However, we are legally obligated to notify your healthcare provider (such as your OB/GYN, MD, DO, or general practitioner) within five days of your initial evaluation. Therefore, we will ask for your general practitioner's information to comply with this requirement.

*You do have the option to decline that we notify your physician. This option is define on the initial intake paperwork that you will receive once you have scheduled your evaluation.

Payment is expected at the time of service and can be made via cash, card (VISA, MC), or HSA/FSA.
